Nunavut Health Care Plan

Applying for Health Care

To register for the Nunavut Health Care Plan, you must fill out an Application for Nunavut Health Care Coverage form.

You can download your application form below.

Application forms are also available at all community health centres and nursing stations in Nunavut, the Qikiqtani General Hospital in Iqaluit, and at the Health Insurance Programs Office in Rankin Inlet.

New to Nunavut

Steps:

  1. Print and fill out the application form: 

    A-508 New to Nunavut Health Care Coverage Form
  2. Provide two clear copies of identification (birth certificate or valid passport or valid immigration documents (mandatory) plus one secondary document, previous healthcare card, employment ID, valid drivers license – for further information please contact @email or 1-800-661-0833.

  3. Proof of residency document that confirms your intention to make NU your main place of residence for over 12 months and that you are not a temporary or rotational worker –  letter from employer, job contract indicating term – for further information please contact @email or 1-800-661-0833.

  4. Email your form along with clear copies of all necessary documents to @email or mail your form along with the necessary documents to the address listed below this chart.
     

Newborn Baby

Steps:

  1. Download: NHCP- Application for NewBorn - YELLOW
  2. Print and fill out your application (see back of application.

  3. Mail your form to the address listed below this chart

  2. Print and fill out your application (see back of application)

Lost Cards: 

To keep your Nunavut Health Care Card up to date, contact the Health Insurance Programs Office in Rankin Inlet if you:

  • Lose your card
  • Change your name or address
  • Live outside of Nunavut for more than three months for medical reasons, school, or work
  • Move out of Nunavut

The Application for Change of Personal Information form is available online and at all health centres and nursing stations in Nunavut and at the Health Insurance Programs Office in Rankin Inlet.
