Health releases new Qikiqtaaluk patient satisfaction questionnaire 

 

The Department of Health is releasing a Qikiqtaaluk patient satisfaction questionnaire between September 15 and November 1, 2018, in order to obtain feedback from the communities which will be used to improve health services. 

Patients, clients and families can complete the survey at the end of their visit at the hospital or at any of the health centres in the Qikiqtaaluk Region. Health staff will be available to assist patients with the questionnaire and all responses will be confidential. 

If you wish to participate you may: 

  • take a copy of the questionnaire offered to you by a staff member at the end of your appointment; 
  • fill out the questionnaire in the waiting room and return it completed to hospital/ health centre staff in the envelope provided; or 
  • take the questionnaire home, fill it out and mail it back to the Office of Patient Relations in the provided pre-paid envelope. 

The questionnaire will be available in all official languages across the Qikiqtaaluk Region, with plans for release in the Kitikmeot and Kivalliq Regions in early 2019.
