Applying for Health Care
To register for the Nunavut Health Care Plan, you must fill out an Application for Nunavut Health Care Coverage form.
You can download your application form below.
Application forms are also available at all community health centres and nursing stations in Nunavut, the Qikiqtani General Hospital in Iqaluit, and at the Health Insurance Programs Office in Rankin Inlet.

New to Nunavut
Print and fill out your application
Provide two clear copies of identification (birth certificate or valid passport or valid immigration documents (mandatory) plus one secondary document, previous healthcare card, employment ID, valid drivers license – for further information please contact email@example.com or 1-800-661-0833.
Proof of residency document that confirms your intention to make NU your main place of residence for over 12 months and that you are not a temporary or rotational worker – letter from employer, job contract indicating term – for further information please contact firstname.lastname@example.org or 1-800-661-0833.
