Reporting to the Supervisor of Home & Community Care, the Home and Community Care Worker II (HCCWII) will assist clients in activities of basic personal car, and if necessary, household management according to an established client care plan, helping clients to maintain independence and functioning in their home environment. The HCCWII will have frequent contact with the client and family, and observe the client’s condition and any changes in their support network.

The HCCWII will ensure all client, family and program information, records and communication are treated with strict confidentiality. You will carry out client care and /or service plans; prepare nourishing meals and special diets as required including planning means with the client and grocery shopping as required. You will perform light housekeeping duties; assist in activities of daily living, and light laundry mending of clothing. You will record data appropriately and accurately, observe client’s condition, needs and report changes to the Supervisor.

The HCCWII should have excellent interpersonal and communications skills, language and interpretation (Inuktitut required), working knowledge of household management skills, an awareness and understanding of the Nunavut Home and Community Care program policies and procedures. You should be able to work independently with indirect supervision. You should also have working knowledge of aids to assist in activities of daily living such as canes, walkers, lifts and transfer boards.

The ideal candidate will have grade ten plus two years’ experience. Current CPR and First Aid Certificate, Training in Personal Care Skills and computer skills are assets. You should possess or ability to obtain a valid driver’s license within one year of hiring.

Equivalencies that consist of an acceptable combination of education, experience, knowledge, skills and abilities may be considered.

Fluency in more than one of Nunavut’s official language is an asset.

Knowledge of Inuit language, communities, culture, land and Inuit Qaujimajatugangit is an asset.
