Documentation / What Should I Bring When I Go To Apply?
Your
Community Income Support Worker will need certain up-to-date
information at the first appointment and each time they review
your benefits. Please bring:
Your
Nunavut Health Care card
Health
cards for everyone in your household
Your
Social Insurance card
Social
Insurance cards for everyone in your household 18 years of
age or older
Court
orders or legal documents that affect your financial status
(divorce or separation documents, maintenance orders)
Information
on any money that you have received in the past 30 days, including:
Pay-cheque stubs from full- or part-time employment
Employment Insurance or Old Age Security cheque stubs
Income Tax refunds - cheque stub or documentation showing
a direct deposit
National Child Benefit Supplement statement (from Revenue
Canada)
Information about any other money you have received in
the past 30 days, including lottery and bingo winnings
Rent
receipt or bill
Copies
of mortgage/rental agreements
Your
fuel bill
Your
power bill (NPC)
Depending
on what kind of benefits you are receiving, there may be other
documents you Community Income Support Worker will need to see.
